Long-Term Clinical and Radiosurgical Results of Meningiomas Involving the Petrous Area Treated with Gamma Knife Radiosurgery

Abstract
Objective
We sought to determine how well meningiomas in the petrous area are controlled after gamma knife radiosurgery (GKS) and the progress of related cranial symptoms.
Methods
Between May 1997 and December 2006, 91 patients underwent GKS for the treatment of 107 intracranial meningiomas at our hospital. Among a total of 23 patients with meningiomas in the petrous area, 5 were excluded because GKS was used for surgical remnants. The mean age of the 18 included patients was 53.3 (range, 38–76) years. All patients were followed >8 years, and the mean follow-up period was 11.18 (range, 8.16–14.72) years. All cases were reviewed retrospectively, and clinical follow-up data were collected from outpatient records and by phone. Patient symptoms were divided into neurologic and non-neurologic types.
Results
Among the 18 patients, the tumor volume decreased in 13 patients (72%) and was stable in 4 (22%). Initially, 8 patients had cranial nerve symptoms, 4 had non-neurologic symptoms, and 6 were asymptomatic. Among the 8 patients with cranial nerve symptoms, 3 experienced improvement and the rest stayed the same. In our patients, complications such as hearing loss and diplopia occurred in 2 of 9 patients with non-neurologic or no symptoms.
Conclusion
Meningiomas in the petrous area were well controlled in the long-term after GKS. Among symptoms related to cranial nerves, those related to the trigeminal nerve were more likely to improve.
